Forms

If you would like to join the Group I would ask that complete the 'Sign-up' Form, this makes sure I have the correct and current contact details for everyone. Once I know you wish to join I'll give you a call to explain how I am going to set it up, payment, and access etc. I realise some of you may have trouble with the IT, I am currently experimenting with options to make it as easy as possible and I'll help get it set-up once we're sure the best route.

If you are looking at these Forms on a 'phone there are two, one at the top the 'Sign-up' Form, and below that another, the 'AFL Screening' Form. While you will all have filled this in some time ago, this crucial information is filed where I cannot access it. I would prefer, too, to make sure I have up-to-date information on all of you, so may I please ask you fill it in....just run down the Form and press 'Next' ( there are four Sections) ....at the end I ask that you type in your name (to sign it) and date it...then press submit.
