The first step in this process is to fill out your online MedXpress application.
IMPORTANT: I can not stress enough not to put disapproving information on the medical application. Some illnesses cannot be helped, and you must obtain a waiver from your doctor to pass a first-class medical. With that being said, if there are:
Medications you take that you aren't dependent on to stay healthy
and/or
Recent illnesses that you have had that no longer affect you
DO NOT include them in your application. The military does not do well with outside medications and/or certain illnesses, so if one of those two apply to you and the illness/medication you are taking no longer effects you, don't include that information on your application. It could be grounds for an immediate disqualification. Common examples are ADHD medication and early childhood asthma.
After filling out your application for a first class medical certificate, you will have to schedule an appointment with the nearest Aviation Medical Examiner (AME) to complete your first class medical.
The closest AME is:
Leonard J Polinski, MD
Polinski Medical Associates
526 Perry Highway, Pittsburgh, PA 15229
(412) 931-7415
It is important that you schedule an appointment early, before winter break if possible, because if a disqualifying item is found then a possible follow up appointment will be needed.
