During your first appointment, your intake, your therapist will present you with a Professional Disclosure Statement, or PDS. A PDS is a document that is required to be on file at the State of Michigan's Department of Licensing and Regulatory Affairs (LARA). You will be asked to sign a copy of Lakeshore Guidance LLC's Professional Disclosure Statement at your intake session.
You are welcome to fill out intake forms before your appointment or during your appointment. Having forms filled out before we meet allows us more time to discuss the reasons for your appointment. The intake forms that you will receive have been designed to give your therapist a clear picture of who you are. If you are uncomfortable committing certain things to writing, you are welcome to leave those sections blank. Your therapist will ask about these if the information seems pertinent to your healing.
