Stage 2: Design

Purpose of Stage 2

Specify the practice workflows and other aspects of team-based, systematic integrated care in keeping with the vision, assessments, and strategies defined in Stage 1 Planning.

The Stage 2 Workbook is the design team’s guide to turning the Stage 1 blueprint into an implementation plan for Integrating Behavioral Health and Primary Care (IBH-PC).

The eight steps explain how to introduce new team members to this work, analyze current workflow, select tactics for improving integration, design future workflows, and develop an integration plan for Stage 3: Implementation.

Approaches for working on Stage 2: Design Workbook

There are many effective approaches for conducting design activities in Primary Care. This workbook can be used in two ways:
