SUPPLY OF CONTAINERS / POCT
Requesting Specimen Containers:
Internal Clients: Specimen containers can be obtained from Store Pathology at Level 2, Day Care Centre using indent form (KEW.PS.11).
External Clients: Hospitals within the cluster Zon Selatan Kuching should use indent form (KEW.PS-7).
Operating hours at Store Pathology:
Time Sending Indent Form
Every Monday and Wednesday from 8.30am – 12.00pm / 2.30pm – 4.30pm
(Including Public Holidays from 8.30am – 12.00pm / 1.00pm - 2.30pm)
Time to Collect Item
Every Tuesday and Thursday from 8.30am – 12.00pm / 2.30pm – 4.30pm
(Including Public Holidays from 8.30am – 12.00pm / 1.00pm - 2.30pm)
Indent Form Requirements:
The form must be fully completed with clear item descriptions, quantities requested, and include the indenter’s stamp and signature.
Quantity Approval:
Quantities are approved based on current stock levels and usage.
Arrangements for Off-Hours Supply:
For supplies needed outside the specified times, please contact Store Pathology in advance to arrange an alternative pickup time.
Reporting Issues:
Report any issues with the supply of specimen containers to the officer on duty for prompt resolution.
7. Justification of Requests:
Requests must be justified by actual usage. Excessive requests may cause storage issues and lead to item expiry.
8. Collecting Bulky Items:
Use a trolley or large plastic bag to safely transport bulky items.
9. Verification Upon Collection:
Verify that the items and quantities supplied are correct. The collector must provide a signature and stamp upon receipt.
10. Stock Inventory Record-Keeping:
Maintain accurate stock inventory records, such as using a Tally Card. We reserve the right to inspect inventory records for any discrepancies or unusual requests.
11. Handling and Storage: Ensure safe handling and storage of specimen containers according to product specifications.
12. Reagents and Other Supplies:
For reagents, consumables, or other supplies not listed in Store Pathology, contact the relevant unit in the Pathology Department.
