Registration Forms

If you have not already registered, please complete the registration form below. ALL students, new and returning, must complete the registration form.

Before being officially registered,  each of the following forms Must be completed

To register, please 

1. print out each form 

2. Fill them out and sign them (preferably written in blue ink)

3. scan forms or take photos

4. email the forms as attachments to homewellacademyga@gmail.com
