The residents we serve are not limited by county lines. The work done by the Winnebago County Health Department and other organizations is in collaboration with organizations across many cities and counties. Given the robust partnerships between Winnebago, Calumet and Outagamie Counties, and with a commitment to fiscal responsibility, efforts were combined across the three counties to work together on conducting the community health assessment. These counties are often referred to as the Tri-County Area.
The Winnebago County Health Department (WCHD) is proud to have excellent partnerships with local health care systems and other health departments. WCHD also partners and works collaboratively with community coalitions, groups and community organizations such as the Partnership Community Health Center. This health center provides the area with affordable dental, medical and behavioral health services for the medically under-served living in the region. Many of these partnering organizations engage in assessing the health of the communities in the region.
The Fox Valley Community Health Improvement Coalition (FVCHIC), a group comprised of local health departments and health care systems, worked together to streamline the process for conducting the Community Health Assessment. The Fox Valley Community Health Improvement Coalition includes:

• Advocate Aurora Health
• Calumet County Health Department
• Children's Hospital of Wisconsin - Fox Valley
• City of Appleton Health Department
• City of Menasha Health Department
• Outagamie County Public Health Division
• Thedacare
• Winnebago County Health Department
Based on the collective resources and with the support of the State Department of Health Services (DHS), these partnerships were able to provide a shared Community Health Needs Assessment (CHNA) in 2018. Together, these partners collected data after they identified what data needed to be collected and data collection tools. The purpose of the collaboration was to collectively:
• Determine the current community health needs by county and public health jurisdiction of the region
• Gather input from people who represent the broad interests of the community
• Gather input from community residents
• Obtain information about the assets and resources that exist in our county
• Identify significant community health needs
The process of collecting data and working together are recommended steps of the Take Action Cycle from the County Health Rankings and Roadmaps, which is a model adopted by many other state and county organizations in Wisconsin.
Various quantitative and qualitative data informed this Community Health Assessment.
Local quantitative data of adults was collected through phone surveys (Winnebago County Community Health Survey) and youth data was collected through a survey administered at schools (Youth Risk Behavioral Risk Survey or YRBS).
Qualitative data was collected through a variety of ways ranging from face to face interviews with local stakeholders and community members; and facilitating community conversations with groups of residents. An effort was made to connect with residents most impacted by health disparities such as low income residents, youth of color and others.
Other core data sets such as census bureau data was also used to inform this county health assessment.
