Required Documents for Continuing Education Reimbursement

Provide the Training and Upgrading Fund with the following documents:

• General Enrollment Form. Please complete all 4 pages.
• Verification of Employment Form. Please include Employee ID number. A Training Fund representative will request employment verification from Human Resources.
• Proof of program completion.
• Program description including cost. (i.e. brochure, flyer, or registration form)
• Final invoice showing payment in full. Please make sure your name, school name, tuition and fees are all listed.

Next Steps:

• Email of fax your application and supporting documents to your Training Fund representative.
• After submitting an application you will receive an acknowledgement of receipt email. Please allow 24-48 business hours for a response.
• CE Reimbursement applications with supporting documentation are processed within 6-8 weeks of receipt.
• If your application is approved, a reimbursement by check will be sent via regular mail and should be received within 7-10 days from the processing date.
