Required Documents for USA Visa

1. Original Passport.

2. 2 Photos 50 X 50mm, white background, 70% face cover.

3. Income Tax / Form 16 for last 3 years

4. Bank Statements / Passbook updated.

5. Company registration proof if self-employed eg. Shop Act / MOA/ Deed.

6. Last 6 months salary slips - if employed

7. Employment contract.

8. If student School/College ID card.

9. Retire letter/ Pension Order - if retired.

10. Air tickets - if any

11. Any other financial documents like FD, Shares, Mutual Funds, NSC, PPF, Property papers or any other financial documents.

Documents if Invited by Friends/Family:

1. Passport & Visa copies.

2. Invitation letter.

3. Bank statements for last 6 months updated. - if sponsoring.

4. Salary slips for last 3 months.- if sponsoring.

5. Affidavit of Support I-134 - if sponsoring.

Documents if someone else is sponsoring the trip:

1. Sponsor letter with signature ID proof.

2. Income Tax / Form 16 for the last 3 years.

3. Bank Statements / Passbook updated.

4. Any other financial documents like FD, Shares, Mutual Funds, NSC, PPF, Property papers or

any other financial documents.
