Arranged files such as the SSS, Pag-Ibig, Phil Health, Resigned papers, Request letters, Maternity leave, Pay slips, exit clearance, and other files. I sorted similar files, combined them, and put them each in a folder.
Sorted the dates of the request letter of employees starting from 2018 to 2024.
Removed the documents of resigned employees from their 201 file and transferred in brown envelope.
DAY 2: JUNE 13, 2024
Organized and sorted 201 files.
Sorted the Landbank application slips of employees.
Cut-out the pay slips of employees.
DAY 3: JUNE 14, 2024
Helped to bring the materials to the Triage area for the Father's Day event.
Assisted giving foods to the patients.
Took photos of patients and employees during the Father's Day event.
Prepared the salary of employees.
Transferred the Landbank account number of employees into their software.

Sorted in alphabetical order the Probationary Service Agreement, Regular Employment Contract and Job Description of the employees.
Categorized chronologically the Sign-up Salary Advance forms and the certifications of the employees.
Inserted documents in 201 files.
DAY 5: JUNE 19, 2024
Categorized new copies of resume based on employees' division.
Inserted the resume in 201 files.
Assisted the employees to get their Probationary Service Agreement, Regular Employment Contract and Job Description.
DAY 6: JUNE 20, 2024
Timekeep more than 20 DTR of employees.
Listed the names of employees who don't have labels in their 201 files.
Prepared and designed the materials needed for the Pride Month event.
Answered calls from different division.
DAY 7: JUNE 21, 2024
Helped in facilitating the Pride Month program.
Stapled 50 copies of Employee Evaluation Form.
Inserted the Probationary Service Agreement, Regular Employment Contract and Job Description of employees to 201 files.

Cut out the pay slips of employees and arranged alphabetically.
Observed the recruitment and selection process.
Summarized and made notes for the minutes of the interview.
DAY 10: JUNE 26, 2024
Cam scanned 60 pages of HEA files from January to March.
Sorted the payroll slips and cash basis payroll.
Separated the files of resigned employees and placed the documents in envelope.
Compared and checked the list of the labels to the 201 files for accuracy.
Changed the old labels in 201 files.
DAY 11: JUNE 27, 2024
Inserted additional documents in 201 files.
Continued to change the old labels in 201 files.
Answered calls and communicated it to the HR staff.
Helped in organizing the employees' salary.
Helped in cutting out the designs for Turnover Ceremony event.
DAY 12: JUNE 28, 2024
Painted the letters for the decoration of the Turnover Ceremony.
Bought additional paints and picture frames in Pandayan Bookshop.
Made an alphabet labels to categorize 201 files in each letter.
Helped in decorating the stage for Turnover Ceremony.

DAY 13: JULY 1, 2024
Assisted the unit heads, doctors and regular employees to their indicated chair during the Turnover Ceremony.
Separated the files of resigned employees and placed the documents in envelope.
Inserted Probationary Service Agreement of 25 employees in their 201 files.
Photocopied more than 200 copies of Employee of the Month Nomination form
Edited the Probationary Service Agreement of 9 newly hired employees.
Signed the leave form of the employees in the absence of Ma'am Rhianne in the office.
Inserted the PMRF of employees in 201 files.
DAY 14: JULY 2, 2024
Listed the HMDF, SSS, TIN and PHIC of employees for the database file.
Inputted the information of employees in database of the HR.
Edited the Notice letters. Changed the names, dates and the rules they violated.
Dry sealed the certification letters.
Organized the Probationary Service Agreement of new employees.
DAY 15: JULY 3, 2024
Inserted the Code of Conduct , Acknowledgement, Job Description, Orientation notes, Probationary Service Agreement and nicknames in envelope.
Made new 201 file folder for new employees.
Filed and organized the Employee of the Month Nomination form.
Assisted Ma'am Marjorie in finding the One Covid-19 allowance files.
Photocpied and files the HEA reports from January to May.
DAY 16: JULY 4, 2024
Continued to organize inserted documents in the 201 files of new employees.
Delivered the Employee Performance Evaluation to PhilHealth office, Cashier and Billing Office.
I timekeep and listed the who have emergency, sickness and maternity leave.
Rechecked the employees name for accuracy in the Government Mandated Contribution file.

DAY 18: JULY 8, 2024
Cam scanned the authorization letters of employees.
Helped to find waiver letters in 201 files and listed those have no waiver.
Shredded the Regular Employment Contract.
Edited the Regular Employment Contract in MS Word.
Changed the labels in 201 files.
DAY 19: JULY 9, 2024
Organized the Probationary Service Agreement.
Counted the Appointment letters and inputted the data to their software.
Helped to find the receiving copies of One Covid-19 Allowance from January to September 2022.
Cut out more than 100 pages of Overtime Claim forms and Request for Overtime forms.
Edited the Probationary Service Agreement in MS Word.
Routed all the offices for the pre-registration of dog show competition.
DAY 20: JULY 10, 2024
Helped Ma'am Rhianne to write her name in the signatory section the Employment Regular Contract.
Sorted the appointment letters, Probationary Service Agreement and Job Description of the employees.
Assisted the employees to sign and get their Probationary Service Contract.
Edited the certification awards of employees in Canva.
Routed all the offices for the pre-registration of dog show competition and follow the forms of Employee of the Month Nomination.
Delivered the communication letter to the medical director office for receiving.
Listed the Job Description and TIN number of employees for notarization.
