If you are being paid for your work, and you experience an injury, it is a worker's compensation claim; this includes students who are on work-study calls. The reporting process for these injuries is to call the CorVel Worker's Compensation Line
at 833 - 400 - 7222.
A nurse will answer the phone and triage you, offering medical advice and next steps forward for treating the injury and returning to work. A report will be generated that is shared with the Safety Office, describing the incident.
Anna Glover and/or Kelly O'Loughlin will reach out to you to ensure you have everything you need for recovery, and to inquire about the incident as needed.
If you are injured while not being paid, as a student working on Professional Work Assignment (PWA) or otherwise in our spaces such as during class time,
it is a General Liability claim.
Please reach out to the Director and Assistant Director of Theatre Safety at anna.glover@yale.edu and kelly.oloughlin@yale.edu to let us know what happened, and fill out the form below.
When an audience member or visitor in our community is injured, a Patron/Visitor form needs to be completed.
Fill out the form here, and email the completed form to the Director and Assistant Director of Theatre Safety at anna.glover@yale.edu and kelly.oloughlin@yale.edu, as well as Assistant Director of Audience Services at shane.quinn@yale.edu.
A Near Miss is an event that had the potential to cause, but does not actually result in, injury or accidents. A "close call" would be another term you may hear. Some examples would be a slip in a puddle but you did not fall or get hurt. Or, loose hardware falling from the grid, but does not hit anyone on the way down.
Please report these moments to the Safety Office using the form below. The more Near Misses we catch, the more likely we will be able to prevent injuries and accidents in the future!
