Whether you are already approved for FAMLI, or still in the application process and awaiting approval, following the steps below when notifying United that you will miss work are best practice and help prevent errors in administration. However, nothing prevents you from applying for FAMLI after a "regular" sick call and within 30 days of your first day of work missed (90 days in extenuating circumstances, such as not being aware of FAMLI).
Always give as much notice as possible .
Call 800-FLT-LINE and follow prompts for regular sick leave.
At the start of the call, get the NAME of the FAST representative.
Next, state that you are "Denver based and placing yourself on "Colorado FAMLI Leave for Self/Family Member/SAFE or other".
The absence should continue to "roll" until you call in well by calling FAST when you are ready to come back to work.
The codes that should be placed on you Master Schedule will be:
FCS (CO FAMLI for Self) Unpaid and conditionally rolls in the Master Schedule. If your absence is 30 days or less, you must call yourself OFF sick leave when you are ready to return to work. More than 30 days may require a Return to Work Absence Certificate.
FCE (CO FAMLI for Family Member/Designated Person) Unpaid and conditionally rolls in the Master Schedule. NEW: You must call FAST to remove the CO FAMLI absence code and update your status when ready to return to work.
Following the above steps should ensure your FAMLI is administrated correctly by United, including ensuring you are NOT paid from your sick bank (since FAMLI is paid by the State). If for some reason you are paid from your sick bank, you will have to pay this back to UA so maybe set it aside as you will be contacted for repayment.

You must file your claim with the state by accessing the Colorado My FAMLI+ online portal. Your claim should be filed within 30 days after your first day of work missed.
Any FAMLI claim filed between 31-90 days after the first date of absence will be considered if the Claimant also establishes good cause for the delay. Reasons for filing late after 30 days may include:
you were never notified by your employer about the FAMLI program
you are a new hire and were not advised individually at based orientation about FAMLI (for orientation dates Jan 1, 2024 or later)
you were not notified about the FAMLI program within 5 business days of notifiying your employer of a qualifying event
If your planned leave is for Parental Bonding, notice to the employer should be given 30 days prior, if possible.
When asked, United's Employer Identification Number (EIN) is 74-2099724.
When you initially apply for FAMLI, under Employer Info, you will be asked to enter your Weekly Work Schedule. The easiest way to do this is to take your average monthly hours, divide by 30 and enter that amount of hours for each of the 7 days. If you are applying for continuous leave, this will not be an issue. If you are applying for Intermittent leave, you will need to adjust this later when reporting hours of work missed (see below).
For intermittent claims, after your claim is approved and when reporting your hours of work missed, be sure to report Flight Credit hours (not duty hours or time away from base) since it is the flight credit hours for which we are paid.
When reporting work missed after each week, you will need to update your weekly work schedule so that your pay will be calculated correctly. NEW! Here is a video explaining how to report hours missed each week, including how to update your Last Week's Work Schedule for our irregular schedules.
While you do not need to have a My FAMLI+ account prior to calling FAST to notify the company of your need for time off, if you know in advance it's best to get the application and claims process started as soon as possible, and you will need to create a CO FAMLI+ account in order to do that. Remember you have 30 days from the first day of work missed to file your claim, normally.
If for some reason your claim is denied (either for the $2500 earnings requirement or for medical/situational facts that were provided), you can appeal the decision by going to your MY FAMLI+ account and accessing the “appeals” options provided through My FAMLI+. Simply log into your account, navigate to the “Claim Details” page and follow the instructions for an “appeal.” Refer to the “Filing An Appeal” section of the My FAMLI+ User Guide.
Note: My FAMLI+ will route an employee’s appeal through a redetermination review. After that review, My FAMLI+ will allow an employee to select “appeal” a second time, which will serve as a request for a hearing before the FAMLI Appeals Unit.
