Forms
If you completed community service outside of the unit, you must use this form to recieve credit.
Permission Slips are required if you miss instructional time and if you are traveling with the unit and an instructor. (Certain Com.Serv./Competitions)
Transportation Forms are required if you are providing your own transportation for an event that has an instructor. (Certain Com.Serv./Competitions)
Operation Related Forms
Standard Release Forms, Health Risk Screening Forms, Conduct Forms, and Sport Physicals are all required for cadets in the unit.
They must be resubmitted each year.
