New Patient Visits
To schedule NPV:
Send a HIGH PRIORITY message to the CLERICAL POOL with patient's chart attached (if available) and subject line "Add New Patient Appointment"
Ask to schedule patient for a specific date/time, if known, or ask to call patient to schedule
OR
Call the doctor's line ASCOM 584-7425, option #6, and ask RN to help you schedule the appointment [not available over weekends/evenings]
Care Management
Our care manager, Nicole Wachter, is accepting patients with A1c > 9, HTN, or increased utilization. Place a referral to "PC Care Management".
Collaborative Care Mental Health Care Manager
Casey (Cassandra) Williams is our new mental health care manager for Hoxworth and is ready to see patients for counseling services! Additional information was sent in a flyer to your email and will be in the Precepting Room, but here are the referral criteria:
Patients with 1+ chronic illnesses, AND
PHQ-9 and/or GAD-7 of 10 or greater, AND
Age 18+
PCP is managing mental health if medications are prescribed (not Psychiatry)
No other current mental health professional connection.
Refer to Casey by placing a referral to Ambulatory Psych, selecting Cassandra Williams as the provider, and noting "Casey Williams Collaborative Care" in the comment section of the referral.

Come to Financial Aid office at UC prior to appointment or at least 1 hour early on day of appointment
Bring proof of income stream AND proof of residence (they're relatively flexible on this one)
If approved, will need to renew every 3 months.

Chart Correction requests can be submitted two ways:
1. Open Footprints ticket through Self-Service or by contacting the UCH Help Desk at 585-MYPC (585-6972).
a. Include all identifying information: MRN, Patient Name & DOB, DOS, HAR or CSN, and Reason for Request.
b. If possible, provide screenshot(s) showing the incorrect information and the location in the patient chart, so
they can be attached to the ticket for reference.
c. Footprints tickets for Chart Corrections at all locations should be assigned to UCMC Medical Records Team.
2. Email request to UCH Chart Correction Team: uch-chart-correction@uchealth.com.
a. Include all identifying information: MRN, Patient Name & DOB, DOS, HAR or CSN, and Reason for Request.
b. If possible, include screenshot(s) showing the iincorrect information and the location in the patient chart.
Please Note:
There are many clinical issues that Medical Records cannot correct, because the correction must be completed by a clinician. This includes corrections related to Flowsheet entries, Medication Administration (MAR), and issues related to Open or Erroneous Encounters.
