Event Registration

This form provides payment for each vehicle your team is registering at each event. This form allows CUSA the ability to plan for the events and pay for the needs of that event.

NEW: Board approved this new policy.  "The first 7 cars at your school will pay full price for each event.  8th car and higher at your school will only be charged 50% of the registration cost per event" 


Form & Insurance Certificate(s): Administrator & Insurance Certificate Form. (Download button on right of screen)

This form is required by CUSA providing an understanding by your School District of your participation at events - as well as the required "Insurance Certificates" by the various challenge sites in the MidWest.


NOTE: Insurance Certificates... CUSA gets many questions about the Insurance Certificates for the various events. These can be requested by your Director of Business Services in your school district (title varies among schools). Page 3 of the Admin form provides an example with details of this Insurance Certificate.
