Student Permissions
On this page, you will find the various Student Permission forms within ChallengeUSA.
Student Permission Form (CUSA)
This form is required by ALL student participants at CUSA events. This is a ChallengeUSA form.
Driver Permission Form (CUSA)
This NOTARIZED form is required by all students that will/might DRIVE an Electrathon or Supermileage vehicle at a CUSA event. This is a notarized form to guarantee there is parent/guardian permission for the student to drive. Students need to be in 6th grade or higher and must be a member of the local ChallengeUSA affiliated team in order to be eligible to drive.
Track Waivers (Varies)
Some of the events that ChallengeUSA participates in (such as Autobahn and Road America) require every attendee of their track to have a separate "Track Waiver" per their Insurance Carrier. These are loaded on our website when we get them from the tracks - though some times this is short notice as it is our of our control. These MUST be completed by students and adults attending and brought to the track gate when you arrive.
DO NOT send these ahead of time to CUSA.
