Application Steps:
If first time DTA site user, create username and password (save in case management notes)
Use an active email or create a new one that client will actively use
Apply for SNAP through the DTA Website
Provide income & household information
Provide rent, utilities, phone, transportation, and air conditioning expense information
Provide a usable address at which client can check mail regularly (use CDCW if needed)
After you Submit the Application:
Client should be prepared to receive mail within the next week, which should include:
A request for a case management phone-appointment
A medical form to be signed by nurse or doctor (have nurse sign if possible to avoid delays)
A list of additional required documents
Additional Information/Follow-Ups:
Submit required documents by fax, mail, or online through client’s DTA portal
