Context: This is a record of the Measles, Mumps, Rubella vaccine. For most DOE students this will automatically be sent in July with the final transcript.
What to do: If this is still on the To Do list in late July, follow up with the High School or upload Immunizations via filenet
FYI: Immunization shows up in the TO DO and the HOLDS sections
Context: This is a form to show that a student either has proof of a meningitis vaccine or doesn’t want to/can’t get the immunization.
What to do: Click on the link in the To Do, read the document, and either submit proof of meningitis vaccine or select the third option (I have read and understand the risks of not getting the vaccine). (Directions here)

Holds prevent students from registering for classes.

Context: The student needs to confirm their NYC Residency to receive in-city tuition rates.
What to do: Students who graduated from a NYC high school should not face this challenge. If they do, contact admissions or fill out this form.
Context: The student needs to complete an affidavit regarding their immigration status. This information will be kept confidential.
What to do: Fill out and submit this form - recommended to do with an advisor / admissions rep.
Context: This is a hold to speak with an advisor before they can register for classes

At Hostos, the first step is to determine if the student is in College Discovery, ASAP or the Coaching Unit (all other students). Students will receive differentiated advising based on these groups.
Students will get an email from CD if they are eligible. They need to respond and say “not interested” before they can be moved to ASAP.

Allied Health Majors (Nursing, Radiology and Dental Hygiene) are not eligible for ASAP
Students who are not in ASAP or College Discovery are then in the “Student Success Coaching Unit”.

Students will receive an email invitation to attend an virtual College Discovery Orientation from a Hostos CD Counselor. Students must confirm they want to participate after attending the virtual orientation.
One-on-one advisement with a CD Counselor will occur once students complete the virtual orientation and an appointment is scheduled.
Students interested in ASAP should email Ashly Dominguez with EMPLID.
Freshman students will be screened for eligibility. All eligible students will be invited to attend an ASAP Information Session.
After the Information Session, students will meet with a peer mentor to register for English and Math only. Once that is done, they will have a one-on-one meeting with the Advisor to register for the remainder of their courses.
Sign up for Orientation session (advisement happens there)
Students will receive an email from enrollmentservices@hostos.cuny.edu and text message starting in April 2021. Students will be sent a link to register for an New Student Orientation (NSO).
NSO is a group orientation, where you will meet with a Success Coach to register for classes.
At Hostos you get your college email two weeks after accepting the offer of admissions. Be sure to check this email every 2 days!
