ROTATION - Neuroradiology
NORMAL ROTATION HOURS: 8:30 am (following am conference) – 5:00 pm. Faculty covers service during noon conference. Fellows may occasionally work evening shifts (3:00 – 10:00 pm) later in the year.
FRIDAY EVENING SHIFT: 5:00 – 10:00 pm after normal shift on assigned service.
SITE: POD1, occasionally POD5 if no neuro fellow is on service
PROTOCOLING: Protocols are typically performed by the neuro fellow or faculty. You may be paged to protocol STAT/ASAP exams from the ED or inpatient services. Faculty are always available for protocol questions.
WHAT STUDIES DO I READ: Neuro studies (CT and MRI) are read as they populate the worklist, with ED and inpatient studies having priority.
CONTRAST ORDERS: Fellows are paged to sign contrast orders as patients arrive. Faculty cover during noon conference.
CHECKING EXAMS: The neuro fellows and faculty are periodically paged to review studies actively being scanned, as needed. As the year progresses, pediatric radiology fellows may be paged to check neuro studies. Faculty are always available for questions or assistance.
CLINICAL CONSULTS: Teams routinely come to neuro reading room to go over studies. Fellows generally provide the initial review, with faculty assistance as necessary. It is important to refer to the report prior to discussing the case to ensure there are no discrepancies in interpretation or information relayed.
TEACHING: There will often be shadowing medical students or residents rotating through neuro from other services in the hospital. Fellows typically go over cases with these visitors and rotators, teaching basic anatomy and pathology based upon the cases that come across the worklist throughout the day.
NEURO CONFERENCES: Each week, there is a multi-disciplinary neuro-oncology (3:00 pm Wed) and neuroradiology (11:30 am Fri) conference. Fellows rotating on neuro are expected to attend both, but are not responsible for presenting or moderating either of these conferences.
ADDITIONAL: For the Fri evening shift, fellows run the neuro service from 5:00 – 10:00 pm under the supervision of the on-call neuroradiology faculty. Fellows will protocol urgent neuro exams, sign contrast orders for evening cases, and provide preliminary interpretations for ED and inpatient neuro exams. Once a preliminary report is completed, fellows contact the on-call neuro faculty to let them know a study is ready to be staffed. As time permits, fellows then read the outpatient neuro exams or assist with non-neuro urgent exams, depending on faculty preference and the clinical workload for both neuro and the STAT box.
WEEKEND NEURO COVERAGE: The neuro faculty on-call covers the neuro service during the morning and early afternoon each Sat. Sometime Sat afternoon, the faculty will transfer service coverage to the daytime fellow. From that point on through the remainder of the weekend (including Sun), the fellow will protocol and prelim ED and inpatient neuro exams, letting the neuro faculty know when studies are ready to be staffed. If the workload in the STAT box prevents the fellow from providing a timely preliminary report, the fellow can let the neuro faculty know a study is on the worklist and needs to be read, which they will happily do. During these shifts, communication is key.
