When traveling to a tournament (just like when traveling to a school sporting event), there are two forms that need to be filled out:

Transportation Form -- This form is necessary if you are being driven to a tournament by someone OTHER than either your parent or a school bus. For instance, if you are going to a tournament and will be driven by the speech coach in his (or her) personal car, then you need to fill out this form. If a family friend will be taking you to the tournament you need to fill out this form. If you are going to the tournament via a school bus then you do NOT need to fill out this form. This form MUST be filled out for EACH tournament.

Medical Form -- This form only needs to be filled out once, at the beginning of the season.
