Workers Comp 

Worker's Compensation Process: 

To be used when an employee is injured while at work. 

1. Ensure the injured employee is okay. Provide first aid at the restaurant or direct them to seek additional medical treatment from an urgent care clinic or ER immediately if the nature of the injury is severe.

   • Provide the employee with the "Worker's Comp Medical Form" (Link Below) to the employee if they are seeking medical attention. This will ensure their medical bills are sent directly to our carrier, Zurich and processed properly.

2. Complete the First Report of Injury for the injured employee to start a claim with Zurich using the links provided above based on your location.

3. Confirmation emails and claim numbers will be sent directly to workerscomp@bokagrp.com. If this information is needed by the employee, please feel free to reach out.

4. Communicate with the HR team on any scheduled shifts that the employee has missed or expect to miss.

5. If an employee receives medical treatment for their injury, please email a copy of a return to work note to the HR team to ensure the employee can safely return to work. Should there be any restrictions for the employee, this information must also be sent to workerscomp@bokagrp.com. 

   • If an employee has missed more than 3 scheduled shifts, a doctor's note is required to be submitted and reviewed by the HR team before the employee can return to work.

6. Should an employee be eligible for loss time benefits, they will be contacted directly by Zurich. Employees can contact their Claim Adjuster should they have any further questions regarding this benefit.
