You may request that the forms be mailed to you in advance, or you can now fill it out online at http://goo.gl/forms/FFzgg9tB1g.   Be sure to send an email to let me know that you did the online registration as I do not get any notification that it has been filled out.

Fill out the enrollment form and waiver and send with a check made out to Cheryl Brown, at the address on the top of the form, to hold your place at camp. Please note the name of your child and the date(s) of your chosen camp session on your check.

NOTE:  The Enrollment Form and Waiver of Liability are all required to be signed by a parent  or guardian to begin any Camp session.

Parents, please review the camper code of conduct with your child before attending sessions.  

Please be prompt to camp sessions. No refunds are given for missed time.

If we must cancel a session for any reason, including insufficient enrollment, your payment will be promptly refunded.
