A Use of Deadly Force Report (FD-005) must be submitted when an agent intentionally uses force that is likely to cause serious bodily injury or death. This includes the discharge of a firearm at a person, intentional use of a vehicle as a weapon, or any other lethal method used in the line of duty. The report must clearly explain the circumstances that led to the decision to use deadly force, the perceived threat, whether any injuries occurred, and what medical aid was rendered. Details such as involved personnel, witness accounts, bodycam footage, and supervisor notification must be included. All reports of deadly force must be filed within 12 hours of the incident to ensure proper review and accountability.
Please note that a seperate Arrest Report (FD-006) must be filed if the suspect is in FBI custody.
