When you first start working with us one-on-one, we will ask you to complete some forms, which include a Demographics form, Release of Information form, and others. We also may ask for a scan/picture of your insurance card(s). We ask for this information so we can help you as efficiently and as much as possible.
Once completed, please email completed forms, a photo of the front and back of your insurance card, and any documentation (EOBs, denials, etc.) to AKG.Advocacy@outlook.com.
