One of the forms we will go over is a medical screening form. This form has roughly 150 individual questions regarding your current and past health. It's imperative that you understand, ANY QUESTION MARKED YES WILL REQUIRE EITHER EXPLANATION or MEDICAL DOCUMENTATION. This form will be sent to MEPS (Military Entrance Processing Station) where a medical professional will review your answers and qualify or disqualify you.
