Submissions and clearance
Risks may be submitted to HDVI for clearance up to 90 days prior to the proposed effective date. Clearance requirements The following is the minimum required information to clear a risk for your agency:
Completed application HDVI or comparable fleet truck insurance applications; no ACORD applications
For the person who works for or with the fleet and has admin access to the telematics Fleet telematics administrator contact details systems, we require full name, email address and phone number
Exposure history Unit count, mileage and revenue for the projected, expiring and 4 prior terms
Driver list Drivers’ names, date of birth, date of hire, years of CDL experience
Equipment list Year, make, model; if PD is requested, include stated value
Loss summary Number of losses and total incurred for each line of coverage requested segmented by year or the expiring and 3 prior terms (4 years total) (expiring term plus 4 prior terms for fleets with 26+ units (5 years total))
Loss runs for AL only - For the expiring and 3 prior terms valued within 90 days of the effective date (expiring term plus 4 prior terms for fleets with 26+ units)
Other lines loss runs - For the expiring and 3 prior terms valued within 90 days of the effective date
IFTA/telematics mileage Last four quarters of IFTA reports
Financials For fleets with 30+ units, audited within the past year
