Clearance Information for New Applicants

Tuberculosis (TB)

2 negative TB skin tests (PPD). Tests must be at least 1 week apart and up to 1 year apart.

OR

1 negative TB blood test.

*valid for 1 year*

OR

1 negative TB Chest X-Ray and Questionnaire if the TB skin tests or blood test results are positive. A TB Questionnaire will be sent to you via email from your Administrative Coordinator. Please fill out the form and submit it on CORE ELMS.

Influenza (Flu) Vaccine

Valid for 1 year. Required for Rotations A and D.

Hepatitis B (Hep B)

3 vaccinations over lifetime

OR

1 positive titer test. If negative, follow up with 3 HepB booster shots

Varicella

2 vaccinations over lifetime

OR

1 positive antibody titer test. If negative, follow up with 2 Varicella booster shots

OR

History of the disease with a note from your primary care provider

Measles, Mumps, Rubella (MMR)

2 vaccinations over lifetime

OR

Positive MMR titer tests. If negative, follow up with 2 MMR booster shots

Tetanus, Diphteria, and Acellular Pertussis (Tdap)

1 vaccination which expired every 10 years

Dtap, DT, or Td vaccinations will not be accepted

CPR Certification

The course must meet certification requirements set by the American Heart Association in Basic Life Support for Healthcare Providers

CPR certifications from any other organization (e.g. Red Cross, Heart Saver/AED, National CPR Foundation, National CPR Association, etc.) will not be accepted.
