Assessments, surveys, data collection, and information forms are all examples of how Google Forms could potentially be used by teachers.
Something I like about Google Forms is that you can collect data and have it go straight to a Google Sheet.
Another feature is creating a password protected form, so that students can only begin the form when they are given the password.
Introduction Form
The sample form that I included on the left is a Google Form that I made about getting to know students at the beginning of a school year.
I filled these types of forms out in my elementary years of school, but they were always on paper. A Google Form is a fun way to give out this type of form to students and it can include different types of questions.
I used short answer questions, multiple choice questions, and a rating question based on the type of information that I was looking for. That is something I really like about Forms. The question types can be varied and you can even include pictures in the answers like I did in one of the questions.
I also included a couple images to amplify the form. I used an image I made at the beginning of the presentation to welcome students to the class. Then near the end of the form I included an image of a document I created with information about myself so students could get to know me too.
I asked questions that I thought would be important to know about students such as what they wanted to be called in class, their goals for the school year, and how prepared they felt for the school year. I also asked what else they wanted me to know about them. These questions would help me be prepared and help me evaluate how each student was feeling at the start of the year.
Asking students about their goals also gets them thinking about what they want to achieve that year.
I made sure to make all of the questions on the form required to make sure that students actually filled out every question and did not skip any.
Project Sign-Up
The next Google Form I included is a form I made where students would sign up for a project.
I made sure to ask the student what their name was so I know who filled out the form.
This is an easy way to have students sign up for a project and to keep information organized.
