TRAILS INVOICE AND PLATFORM
The TRAILS platform records the activities you have attended or completed. It also provides information about the amount you receive for each. You can earn up to $3,000 each year of the project. At the end of each project year, one invoice is completed for payment, and a new invoice will begin for the next year.

We are now collected paperwork for participants returning for a second year. We collect new contracts and W9 forms each year of the program. If your banking information has not changed from last year, you do not need to submit another direct deposit form. If you are uncertain if your paperwork has been processed, click on the button below to see your paperwork status.
W9 Form and Example
All participants must complete and submit a W9 form in order to be enrolled in the program and paid a stipend. If you need assistance in completing this form, please see the example provided. Download the form, complete and sign, scan, and email the completed form.
Contract
In order to be enrolled in the program, participants must complete the contract linked below. Download, sign and email. The new contract will be published soon.
Direct Deposit Form
All participants must fill out a direct deposit form in order to be paid the stipend at the end of the service year or receive reimbursement for travel. Download, sign, and email. If you have already submitted this form in an earlier project year, and your banking information has not changed, you do not need to resubmit a new one.
School Connectivity Form
This form will assist with Cost Sharing on Connectivity/Classroom/Office usage. The chart in the button to the left is based on calculations covering energy costs, technology usage, resources, classrooms, and equipment not covered by grant funds. In summary, having Professional Program Ethical Standards for educators and participants (including
students) in a training program is crucial for maintaining the integrity of the teaching profession, ensuring
the well-being and success of students, and fostering a culture of professionalism, trust, and accountability
within the education system. These standards help guide teachers in their roles and responsibilities, both in
and out of the classroom. We ask that all participants read these standards prior to signing a project contract.
