Registration Petition Information: Information regarding late adds, deletion of a course from the USC transcript, late change of grading option, late withdrawal, late change of unit value.
Change of Schedule Form (Drop/Add Courses): Complete and submit this form to OneStop to add or drop certain courses after the first week of classes.
Approval to Register with a Course Time Conflict: Complete and submit this form to OneStop allowing you to register for overlapping course with both professors' approval.
Summer Course Work Pre-Approval Form: Complete and submit this form to OneStop for pre-approval of summer coursework completed outside of USC.
Incomplete Course Form: Complete and submit should you need to take an incomplete. Once you have completed the remaining work, contact the faculty member and grades coordinator to issue a digital Completion of Incomplete form.
Professional Leave Form: Complete and submit this form as well as all requested materials in order to gain approval for professional leave

How do I get d-clearance for a course? Go to the Schedule of Classes. Find the course under the appropriate department. Specific instructions and contact information for getting d-clearance are located at the top of the department page.
How do I determine the number of units for my Directed Research and/or Dance Internship? You will discuss the matter with the faculty member overseeing your Directed Research project or Dance Internship. The number of units should correspond to the amount of work you intend to complete.
