Dear Incoming MAT students,
Thank you if you have already sent me your TB clearance letter. If you have not, please note the deadline for this is tomorrow, June 17th or your student teaching placement may be delayed. To access your TB clearance letter please log into the UCI Student Health Center portal (https://osh.chs.uci.edu/) and download your TB clearance letter under the “Letters” tab. Please email me (jhchu@uci.edu) your clearance letter as soon as possible so we can move forward with student teaching placements.
You should also be receiving emails from UCI Summer Session if you are currently not compliant with your overall UC vaccination/immunization requirements. Please note the deadline is the first day of attending courses. For MAT students, this is Tuesday, June 21st.
Students who do not comply with the vaccination requirements by their respective summer session due date (Tuesday 6/21/2022) will be subject to disciplinary action.
The UC policy requires individuals to show proof of vaccination and to keep their vaccination status up to date. This includes COVID-19 boosters for those who are eligible. Students are required to submit proof of their COVID-19 vaccination or have the option to request for exception on medical, disability or religious grounds, or a deferral based on pregnancy in order to be compliant with the UC COVID-19 vaccination policy.
Vaccine documentation should be submitted at the UCI Student Health Center Patient Portal: osh.chs.uci.edu.

You should now have access to the UCI Student Health Center portal to upload your TB Tests results and Immunization requirements. Instructions on how to upload documents and how to comply with the UCI requirements are available on the SHC’s website:

Please check the Student Health Patient Portal regularly to access your status and for secure messages related to medical information/questions use the “New Student Immunization Requirement” message mailbox.
The initial deadline to submit TB Testing documentation is June 1st, 2022. There is a Final Deadline of June 21st for all of your immunization requirements including COVID initial and booster vaccinations.
For student enrolled in UCI’s Student Health Insurance, coverage will begin 6/14/2022. On 6/14/2022, you may begin scheduling immunization appointments at the Student Health Center following these steps:
How to schedule an appointment for Immunizations online:
If you do not have SHIP, you can choose to receive your vaccines with your primary care provider.
1. Log in to the Student Health Portal using your UCInetID.
2. Select 'Appointments' from the menu.
3. Click on 'Schedule an appointment'.
4. Select 'Student Health Center - Immunizations (UC Required/Recommended)'.
5. Select 'Immunization Request (UC/Recommended Immunizations)'.
6. Select Reason Code and enter the name of the vaccines you would like to receive.
7. Click on 'Continue'.
8. Confirm your contact information then click on 'Continue'.
9. Enter your callback number then click on 'Continue'.
10. Set date to start your search. Then click on 'Search for appointments'.
11. The earliest available appointments will be listed. If you would like an appointment on a different date, go to step 10 and enter a new date.
12. Select time and date of appointment you would like to schedule then click on 'Continue'.
13. Read 'Almost There…' screen then click 'Confirm'.
14. Follow prompts to complete your appointment.

As part of your UCI School of Education admissions requirements and in preparation for your student placements, you will be required to submit a negative TB test. If you have a history of a positive TB test, you will need to complete the TB Health Assessment form (signed by your primary care provider) and perform a chest x-ray (dated within 1 year of school start date). In addition, UC mandates that all incoming students do the following: 1) Obtain and upload proof of COVID vaccination and booster; 2) Obtain and upload proof of flu vaccination obtained after 8-1-21; 3) Complete an online TB Risk Screening questionnaire; 4) Obtain and upload proof of four (4) other required immunizations. Compliance with the SOE TB requirement and the UC mandated requirements are monitored by and facilitated through the UCI Student Health Center (SHC). The TB test, for purposes of SOE requirements and required for fieldwork placement during the program, is valid for 3 years and must remain current throughout the program. We will accept valid test results dated from January 1st, 2022.

Your access to the UCI Student Health Center portal in order to complete these requirements should be activated in mid-May so that you may enter information and upload your documents. I will email you again once you have access to the system but we want to make sure you can prepare in advance. The initial deadline to submit TB Testing documentation is June 1st, 2022. Students will need to submit verification of a valid TB test by confirmation from a doctor’s office or confirmation from a lab or clinic. If you do not have a valid TB test on file (dated from January 1st or after), we would recommend you completing this requirement as well as obtaining immunizations through one of these options:
UCI Student Health Center:
For students enrolling in the UC Student Health Insurance Plan (UC SHIP)- The cost of testing and immunizations are 100% covered by UC SHIP and your coverage will begin on June 14th. A referral is required by SHC if you receive TB testing and/or immunizations at any outside clinic or provider’s office. A referral is not required if you obtain these services at an in-network retail pharmacy. Note that CVS Minute Clinic is a clinic, a separate billing entity from their pharmacy and, therefore, a referral is required. Contact SHC’s Insurance Services department at shc-insurance@uci.edu or 949-824-2388 to obtain a referral.
For students without UCI Student Health Insurance-You may still obtain services from UCI’s SHC. However, your fees will be transferred to your campus billing account. You may seek reimbursement from your own plan unless it is Medi-Cal, Kaiser or another HMO-type plan (UCI Student Health Insurance Clinic pricing is here).
Contact your primary care physician’s office for testing and immunizations. Reminder – SHC is the PCP for all students enrolled in UC SHIP. A referral is required if a student wishes to obtain TB testing and/or immunizations from any other primary care provider.
