Parents! Please fill out this form! It helps us keep in touch regarding events and will be used in addition to Parent Square. Communications mostly come via ParentSquare. Please download and keep an eye out for important annoucements and information! You can also send messages to Mr. Devalk via ParentSquare.

BOOSTER DUES
Booster dues are $100, payable via Zeffy
First payment of $50 or pay in full by August 7th
Second payment of $50 will be due by the end of November
The first payment of $50 is crucial, as it covers the student's Dbacks ticket and choir shirt.

Choir concerts are meant to be formal and fun for both your student and the audience. In our program, we utilize uniforms that are given to each student along with a few rules to properly dress for the concert. Every student will be issued a school uniform for performances. The cost of renting the uniform is included in Booster Dues.
The uniforms belong to the Corona del Sol High School Choir Program. Each student (and that student’s parent or guardian) must sign a contract upon check out of the uniform. This form will be issued via ParentSquare.
Girls:
Class dress (you will be fitted and provided with one based on your class)
Bella Voce: Black
Novum: Black
Encore: Blue
Acapella: Red
Jaztecs: Red
Students are responsible for obtaining the following items.
Black flat closed toe shoes
Nude stockings (knee-high or full)
Jaztecs ONLY: Black character shoes. Red spandex (dance type) shorts.
Boys:
Novum: Black dress shirt, black dress pants and grey tie will be provided.
Acapella: Black Jackets and black tie will be provided.
Jaztecs: Black dress shirt and red tie will be provided (if the student already has the black shirt they may wear it).
Students are responsible for obtaining the following items
A Cappella ONLY: White dress shirt
Black dress socks
Black dress shoes
Black dress pants (will be provided if your child does not own one)
For all students!
Hair must be pulled/clipped back out of your face (Please check with uniform managers or Devalk if you have questions about this)
No jewelry allowed except studded earrings
Neutral makeup is ok
Uniform Care
Dresses: Machine wash in cold, remove from washer as soon as it's done to prevent wrinkles. Dry in the dryer on low heat for no more than 5 minutes, then hang to air dry.
IMPORTANT: If you dry the dress on high heat, or for too long, the dress will burn. The dresses are also easily burned if ironed. Either don't iron it at all or use a very low setting and a pressing cloth.
Pants and Shirts: Machine wash in cold water. Dry on medium heat for no more than 5 minutes then hang to air dry. Shirts can be ironed if needed.
Jackets: DRY CLEAN ONLY! Please do not eat while wearing the jacket. This will avoid food stains.
Ties: Ties can be handwashed if necessary and hung to dry. Please do not eat while wearing the tie. This will avoid food stains.
Uniforms will be turned in after the Spring concert. Please bring a change of clothes and check out with Ms. Wright and the Uniform team.
If the uniform is not returned, a balance will be placed on the student's account in the bookstore.

Tempe Union High School District requires all volunteers to get a fingerprint card. To begin the process please go to https://www.tempeunion.org/Volunteer.
There is information on where to get your fingerprint card as well as the volunteer application. This process takes a little while to complete so we encourage you to get started right away. If you already have an Arizona DPS fingerprint card, contact TUHSD human resources to verify that they can accept it.
