All students are required to fill out the Band Tour Medical/Dietary Form. If you have specific dietary requirements, please detail these in the form.
This information will be collated and sent to the catering staff of Chittick Lodge.

Please ensure your child knows what they can and can't eat in terms of allergies.
All allergies or medical conditions that may require treatment over the tour need to be recorded by the medical team.
Please include details in the Band Tour Medical/Dietary Form AND Administration of Medicine Form.
The Administration of Medicine form is to be emailed to Mrs Neeves at GLYNIS.NEEVES@det.nsw.edu.au prior to Band Tour.
Depending on the allergy or condition, you may receive a follow up email from the medical team. If any changes need to be made to your child's information, please email the medical team as soon as possible at bandtour@tps-pandc.org.au.
Any medication required on tour must be advised to the medical team in advance, via the Band Tour Medical/Dietary Form AND Administration of Medicine Form, and then provided to Mrs Neeves on Wednesday morning during check-in.
Medication should be placed in a clear snap-lock bag with the child’s name and clear instructions either written on the bag or placed in the bag as to how and when the medication is taken.
Please note no medications (apart from asthma inhalers or EpiPens) can be left in children's bags. This is for the safety of everyone attending band tour.
Children with EpiPens are required to bring their own on tour. A second EpiPen (which is usually kept for them at school), will be carried by Mrs Neeves. A third, unallocated EpiPen will also be in the first aid kit.
Please ensure Mrs Neeves is aware of your EpiPen when you check in on Day One.
Children who will be taking travel sickness tablets during Band Tour will need to detail their usage in the Medical/Dietary Form AND Administration of Medicine Form.
Children who suffer from travel sickness are encouraged to have a light breakfast of toast and fruit (no milk).
Travel sickness tablets should be taken approximately 30 minutes prior to departure when leaving home.
Please send extra tablets for bus travel on Day Two. These must be given to Mrs Neeves on Day One during check-in and must be accompanied by a permission note and/or included on your medical form.
During the tour, we will have two First Aid certified teachers and one First Aid certified parent volunteer. In the event your child gets sick during the tour, you will receive a phone call from our medical team. We do use our judgement and what the child wants! For example, a small blister with a bandaid will probably not generate a phone call, but a child vomiting definitely will. We deal with all of these on a case by case basis, but in some cases you will be asked to come and collect your child. Please be aware that this phone call could come at any time, and we use the Emergency Contact details provided on your forms to reach you.
