Pre-Program Requirements

Please submit the following to Summer Springboard NO LATER than the due date listed.

• The required forms listed below are now available in the Parent/Student Dashboard. The deadline to complete each form is listed in the dashboard, along with directions on how to submit them.

  • NYSID Student Photo for your Student ID

    • Please remember to upload your photo and complete the linked form.

  • Parents & Student Contract and Waiver

  • Student Authorization Form + Self-check out

  • Photo of Insurance Card

    • Front AND Back of insurance card needed*

    • Learn more about insurance on our website.

  • Copy of Immunization Record

    • Immunization Exemption Form Required for students not fully immunized*

  • Flight Itinerary Form

    • Required only if you will be needing Airport Transfer Service*

    • Upload your Flight Itinerary to the Dashboard- if your flight changes, you must email nysid@summerspringboard.com with your new flight itinerary.

*If you need Airport Transfer Service, you must inform us no later than June 1, 2025, and you must provide your flight itinerary by email nysid@summerspringboard.com
