The required forms listed below will be available in the Family Dashboard starting Winter 2025. The deadline to complete each form is listed in the Dashboard, along with directions on how to submit each of them.
This waiver is sent to families using the email addresses associated with their accounts. It is important to note that the waiver will only be sent to the account holder's and the student's email addresses. Therefore, we recommend using personal email addresses for students instead of school emails to ensure that they receive the waiver.
If the email addresses are incorrect, please update either contact's email address before requesting a signature. Once you request a signature, the emails cannot be changed, and the waiver cannot be resent.
The waiver will be sent to the account holder's email. It is important to carefully review the waiver, as you will be able to sign the document digitally. Each page will need to be initialed, and certain areas will require the parent or guardian's printed name, and signature.
After the account holder has finalized the waiver, it will be sent to the student's email. The student will then need to review the waiver, initial, print, and sign their name. Once both parties have signed the document, the form will be marked as "Under Review" in the Family Dashboard for our support team to review and finalize it.
Once the review is complete, the status of the form will change to "Confirmed."
This form will let our campus team know if you would like your student to check out with staff at the end of the program (or each day for commuters), with or without a parent or authorized person* present.

My Student Can Self-Check Out: A parent or authorized person* does not need to be present for the student to check out with staff.
Residential: Your student can check out with staff without a parent or authorized person* present for them to leave campus. Note: If your student is apart of the Summer Springboard Airport Service, they will be a self check-out student as they will be going with staff to the airport at the end of program.
Commuter: Your student can check out with staff without a parent or authorized person* present for them to leave campus.
My Student Can Not Self-Check Out:A parent or authorized person* MUST be present for the student to check out with staff, and depart from campus. The person picking up the student must be visually present for staff - you cannot be "waiting in the car", "around the corner", etc.
*Authorized person: dictated on the form, is a person who is not an emergency contact, that you give the authority to pick-up your student. This can be a family friend, other students' parent, other family member, etc.

What if I don't have an Insurance Card, or I have universal healthcare where I live?
If your family does not have a medical coverage provider, or live where healthcare is universal, then medical travel insurance will be required. The policy should cover the days of the program, including the travel days. The coverage policy that is selected will need to be uploaded in this form section in place of the insurance card

This is a PDF upload in the Family Dashboard. If you do not have a copy of the records, you may use the template in the Family Dashboard, in the submission section of the Immunization Record, to fill out.

What if I need an exemption? Please reach out to your Summer Springboard Admissions Coordinator via email, and we will be able to send you a form to fill out.

Required only if you signed up for the Airport Service
If you are not using this service, you do not need to submit anything for this. An "Incomplete" status will let our review team know that no submission is required for this form.
There are two items to complete for this form: the Flight Itinerary Form and an Original Copy of the Flight Itinerary from the airline.
Flight Itinerary Form: This is a Typeform submission on the Family Dashboard. This will ask questions regarding the full name of the student, arrival/departure time, the arrival/departure airport location, contact information, and more.
Flight Itinerary:This must be the original flight itinerary from the airline. We need to have as much information as possible for your traveler to best support them in their air and ground transportation. This will include flight number, arrival/departure, confirmation number, etc.
Once the items are submitted, the form will be marked as "Under Review" in the Family Dashboard for our support team to review and finalize it.
When the review is complete, the status of the form will change to "Confirmed."

There are no specific forms required for our courses at this time.
Students are required to download the Reach Student Life Management app before arriving on campus. Instructions to set up your student's account will be sent out before your program's start date.
