if your student needs (possibly needs) medication during the school day (study trips, far east events, etc), to include over the counter meds (ex: tylenol, motrin, zyrtec, etc.) or prescriptions (birth control, epipen, inhaler, etc):

• H-3-2 Medication During the School Day: To be completed and signed by Primary Care Physician and signed by parent. A NEW form MUST be completed every school year.

• H-3-9 Student Retention of Medication : To be completed and signed by physician and parent if STUDENT wants to carry prescriptions or over the counter (OTC) on self (EpiPen,Motrin, Benadryl, Zyrtec, etc.) during school hours or on study trips (sports, Far East events, etc.). A NEW form MUST be completed every school year.
