ASTHMA

If your student has asthma please complete the following forms:

• H-8-4 Asthma Information Request Form: To be completed and signed by parent. ALL students with allergy need this on file. One form is sufficient for duration of enrollment. If there are changes to condition a new form must be completed.

• Asthma Management Plan: Completed and signed by Primary Care Physician and parent. A NEW form MUST be completed each school year.

• Form 6000.01-F5 Medication During School Day: If medication is required during school hours, to include study trips (sports, Far East events, etc.). To be completed and signed by physician and parent. A NEW form MUST be completed every school year.

• Form 6000.01-F7 Student Retention of Medication: If STUDENT wants to carry medication (Inhaler, Zyrtec, etc.) during school hours or on study trips (sports, Far East events, etc.). To be completed and signed by physician and parent. A NEW form MUST be completed every school year.
