Form 6000.01-F7 Allergic/Anaphylactic Information Request: (completed and signed by parent) ALL students with allergy need this on file.

Allergy Action Plan (completed and signed by Primary Care Physician and parent) ALL students with allergy need this on file. A NEW form MUST be completed every school year.

FOOD ALLERGY: Nutritional Plan (completed and signed by Primary Care Physician and parent). A NEW form MUST be completed every school year.

Form 6000.01-F2 Mediation During School Day: If medication is required during school hours, to include study trips (sports, Far East events, etc.), medication held in nurse's office. To be completed and signed by physician and parent. A NEW form MUST be completed every school year.

Form 6000.01-F5 Student Retention of Medication: If STUDENT wants to carry medication (Inhaler, Zyrtec, etc.) during school hours or on study trips (sports, Far East events, etc.). To be completed and signed by physician and parent. A NEW form MUST be completed every school year.
