Treatment facilities (other than practitioners’ offices) used for the provision of medical and psychological services must belong to one of the following groups and be licensed and/or certified in accordance with the Colorado Department of Public Health and Environment (CDPHE):
Hospitals accredited by the Joint Commission on Accreditation of Healthcare Organizations and licensed by CDPHE;
Medical laboratories certified and in good standing as per surveys conducted by the Health Facilities Division of CDPHE;
Freestanding or hospital affiliated health centers certified as Federal Qualified Health Centers by the U.S. Department of Health and Human Services;
Health clinics located in rural areas (Rural Health Clinics), which have been so certified under Medicare;
Federally designated community health centers (CHCs);
Community health clinics certified as community clinics by the CDPHE;
Health Maintenance Organization facilities accredited by the National Committee for Quality Assurance (NCQA) and licensed as such by the Colorado Division of Insurance;
Ambulatory surgery centers (ASC) certified as such by Medicare and licensed by CDPHE; or,
Nursing homes licensed by CDPHE as an Intermediate Health Care Facility (IHCF) or as a Nursing Care Facility.
