If a vocational service is scheduled but not provided due to a cancellation with less than 24 hours’ notice or a participant “no-show,” DVR may authorize a cancellation fee of one-half of the allowable fee for the scheduled service or $150.00, whichever amount is lower. This amount is the maximum allowable reimbursement for missed or cancelled appointments and it is not automatically authorized.
A cancellation payment may be approved only when all of the following conditions are met:
The provider requests the payment.
The provider sent the participant an appointment reminder at least 24 hours before the scheduled appointment.
The provider notified the rehabilitation counselor at the time the appointment was missed or cancelled.
The provider submits documentation with the invoice confirming that the participant cancelled with less than 24 hours’ notice or did not attend the appointment.

Pages 1 to 10 - $18.53 maximum
Pages 11 to 40 - $.85 per page
Pages greater than 41 - $.57 per page
Review or edit in-file records is necessary prior to copying, if necessary - $10.50 maximum
Narrative medical report based on in-file records - $42.00 maximum
Records copied from microfilm - $1.50 per page
Records provided on a CD - $25.00 or the above rates, whichever is less
Postage - usual and customary rate
