ACCIDENT PROCEDURES
Student Injury
Call 911 (477-5533 Dispatch) if needed
Notify Parent
The witness of the accident is to complete a Student Accident Information Form and fax it to Central Office. Fax: Attn: Mark Thomas at 502 477-3259 (no cover sheet)
If the student is seen at a doctor or hospital for their injury and the parent requests:
School Authority: Please complete Section II of the Nationwide K&K Claim Form. Make a copy for the student file and give to parent along with completed Accident Report.
Parent/Guardian: Please complete Section I of the Nationwide K&K Claim Form.Parent will need to fax both reports to the given number in the claim form.
