MEDICATION
If your student will be taking any medication during school, the following must be completed:
SCHEDULED AND OVER-THE-COUNTER MEDICATION
If the medication is scheduled, please have a physician complete the medication authorization form. A parent/guardian signature is needed as well. This form can be accessed below.
Medication must be in its original container and brought in to the nurse by a parent/guardian. Pharmacies will give you an extra bottle with a label if needed for school.
If the medication is over-the-counter, the medication authorization form must be completed, a physicians signature is not required unless the medication is needed for more than three consecutive days. A parent/guardian must still sign the form.
EMERGENCY MEDICATIONS
If your student will need an emergency medication at school, a care plan as well as a medication authorization form must be completed by the physician and signed by a parent/guardian. The medication can be kept with your student if the physician indicates on the authorization form. Care plans can be accessed below.

Diabetic students will need student specific orders from their endocrinologist, these orders will include permission for their insulin and emergency medication.
