IMMUNIZATIONS

Kentucky mandates every student have a current Commonwealth of Kentucky Immunization Certificate to attend school. A current immunization certificate should be on file at the school within two weeks of the child’s enrollment. (702 KAR 1:160, formerly 704 KAR 4:020) These same requirements also provide exemptions from these immunizations for both medical reasons and religious beliefs.

By the time your student is ready for kindergarten entry, they should have received the following vaccines, the recommended vaccine schedule can be accessed here:

  • DTaP (Diptheria, tetanus, acellular pertussis) 5 doses total

  • HepA (Hepatitis A) 2 doses total

  • HepB (Hepatitis B) 3 doses total

  • IPV (Inactivated Poliovirus) 4 doses total

  • MMR (Measles, mumps, rubella) 2 doses total

  • VAR (Varicella) 2 doses total, or documentation of previous disease

**IMPORTANT**

A Tdap booster and Meningococcal ACWY vaccine are required when your student turns 11 years old.

A second Meningococcal ACWY vaccine is required when your student turns 16 years old.

Letters will be mailed out periodically to parents/guardians of students who are not compliant with their immunizations, or students who will be due for immunizations soon. Please contact your primary care physician to schedule an appointment to get your student compliant.
