Kansas State High School Activities Association requires a Pre-Participation Physical Evaluation (PPE) annually and shall not be taken earlier than May 1 preceding the school year for which it is applicable. Physical Examinations are required by all students who wish to participate in athletics in grades 7-12 and cheer & dance in grades 9-12. 

You will find the required documents below for you to download and return the South Athletic Office.  The Pre-Participation/Concussion/Transportation forms must be on file in the athletic office before an athlete may participate.
The PPE should be taken to your healthcare professional to be completed.

In addition to the physical forms, athletes and their parents/guardians are required to complete the High School Packet found online in Skyward. These forms must be completed by the student athlete's first day of tryouts. If either parent/guardian or student have issues with Skyward, please contact the counseling office.
