REQUIRED Medical Forms for Summer Camp

Hello Everyone,

Please follow this link to the BSA Medical Forms. They are REQUIRED for Adults and Scouts Attending Summer Camp.

Parts A, B & C are REQUIRED for Summer Camp. Requirement of Part C is best described on the first page of the form. Please read that section. Part C is to be completed and signed by your certified health care processional.

I will accept all completed forms for those going to camp. Please keep a copy for your records.

Summer Camp is very strict about these forms and will turn away anyone who does not have them. When entering camp it is my hope that I will have the complete collection to present to the health officer.
