RNs take responsibility for the evaluation of practice based on agreed priorities, goals, plans and outcomes and revises practice accordingly.
The registered nurse:
7.1 evaluates and monitors progress towards the expected goals and outcomes
7.2 revises the plan based on the evaluation, and
7.3 determines, documents and communicates further priorities, goals and outcomes with the relevant persons.
An evidence-based approach to assessing a care plan can help determine whether certain elements of the plan are working well, inform decisions about the effectiveness of the service, and consider changes that can be considered to improve service delivery.
I practised take a whole patient load and participated a lot with medication distribution, dressings, fall risk assessment, post fall management, communication with patient and patient's family, progress notes, patient's referral, teamwork with doctor and other health professionals. With those process in my mind, I developed a big picture of my nursing care, and it was better for me to evaluate the care plan as well as my own nursing ability.
