Here are a number of files and documents that may be needed at different points of the year.
Outbound Forms and Resources
These are the club level forms you will need as a YEO.
1) The Club level application should be given to the student, and when you receive it back you can schedule your interviews
2) Suggested interview questions for the student...you can use your own questions as well.
3) Suggested interview questions for the parents...you can use your own questions as well.
4) Suggested rating scale to help your club assess the student
5) Form for 3 Host Families ... to be found and listed by the student and student's family.

Note about the 3 Host Families. If you think your student will do well in the interviews, you can give this to them right away. They do not need to find families who will commit to hosting, but they do need to find families who will seriously consider committing over the next 3 years ... this can be of great help to your club when trying to find host families, and it also benefits the student's family by allowing them to deepen their relationship to the program, and also not need to host themselves if they are not ready to do so at this time.
Inbound Forms and Resources
These are forms you may need to track and push forward your progress on setting up an Inbound experience. They are not for the Inbound students themselves, but for you in organizing the Host Families and the arrival of the Inbounds, as well as policies on tracking them through the year
1) Club Commitment Form (InboundCommittmentForm20##_ForYeart20##-## ) due about 14 months or more before the student arrives, this form starts your process with the District, and helps Central States know who many students we will be able to exchange. $1500 must also come with the form. (it should be updated each year, but the only changes are to shift the printed year by 1)
2) School Enrollment Form (SEF). This is a pre-committment from the school that they have agreed to have the student attend. It is not part of the "school's" enrollment process, which will vary by school and is typically done in August, but it is a form that is needed in order for the student to apply for the visa. Target date for completion: February or before (for an August arrival).
3) Endorsement (Guarantee) Form (Endorsement_GuaranteeForm_20##_D6490.pdf). This is the form critical for the visa process. You will need to make sure everything is TYPED, even the dates as much as possible, and the only wet ink should be the signatures. Target date for completion: February or before (for an August arrival)
Volunteer/YEO/YEC
No Paperwork is now needed to bring a new volunteer on board. Whether as a YEO, a YEC or another Rotarian Volunteer who will be having a student overnight, they can all apply by clicking the link to the left called "Apply to be a YEO, YEC or other Volunteer".
The background checks for families or volunteers are now done by InCheck, and the District Chair or District Coordinator will send an email to them which has a link to get them started. Note that all members of the family will need to take the NAYEN youth protection online class.
