There are specific times when a Disqualification Form must be submitted to the PIAA.
Below are some helpful reminders and sample Disqualification Form templates that can be used when required to submit a Disqualification Form.

Provide a clear, concise and factual account of what happened
Be Factual (Accurate)
Include all relevant information to identify the persons involved
Do not give opinions or recommendations
Get Right to the Point (Short and Simple)
Confirm the details in the report with all other officials, (if needed)
Disqualification Form Templates
Sample 1
At the 38th minute of the first half, # 25 of Mount Union HS (John Banks) committed a reckless foul against an opponent. The player was shown a yellow card. As the player was leaving the field, the player used foul and abusive language towards me; therefore, was shown a red card. The head coach was informed a Supplemental Disqualification Form would be submitted.
Sample 2
At the 22nd minute of the second half, # 6 of St. Martha’s (Brittany Gallagher) was shown a red card for committing violent conduct for punching an opponent # 18 of Williams HS (Amanda Robinson).
Sample 3
During the game, the head coach from Wallington HS (Mr. William Tell) was arguing calls against his team. After asking the coach on several occasions to control his behavioral, Mr. Tell was issued a red card for foul and abusive language toward an official. The coach was informed the Disqualification Form would be a Supplemental Disqualification.
Additional Note
The supplemental disqualification rule is for any coach or player ejected from a game for:
confronting, contacting or addressing a coach, player or official using foul or vulgar language
ethnic or racially insensitive comments or physical contact.
The player or coach would be ejected from the game and then suspended for the next two games.
IMPORTANT
There may be times when a Disqualification Form would not be submitted to PIAA; however, an issue occurs, and you should notify your league assignor.
The following events should be reported:
Referee Abusive/Assault by spectators.
Referee Abusive/Assault by Team Personnel / Player(s)
Referee Intimidation by spectators.
Referee Intimidation by Team Personnel / Player(s)
Note: It does not matter if the intimidation or harassment occurs on or off the contest site.
