There are several forms that I must have completed and in my possession before I can assume the responsibility of caring for your child. This includes basic enrollment and health information. You are required to keep me informed of any changes so that current information is always on file. Medical records must be renewed every year.
A nonrefundable deposit equal to one week of childcare costs, is due at time of enrollment, along with your first week’s payment. However, please do not give me the deposit until you have signed the Parent-Provider Contract/Enrollment application and thoroughly read the parent hand book. Your child’s spot is not secured until I have received both the deposit and the signed Parent-Provider Contract/Enrollment application, unless other arrangements have been made. The deposit will be credited towards the last week of your child’s enrollment.
