Optional: Explore Your Own Family Health Portrait

Has your mother or sister had breast cancer? What about a uncle you rarely see; did you know he had colon cancer before the age of 50? Or that your cousin is diabetic even though he runs marathons? Knowing your family history and what diseases you may be at risk for is one of the keys to prevention. Did you know that Thanksgiving is also National Family History Day? Families often gather for the holidays, and this is a great time to at least start this conversation. As you consider how assessing an individual’s health is essential to developing a health plan, think about your own health, your immediate and extended family’s medical history, and what your health portrait might look like. What is your risk? What is the risk for your children or grandchildren? As an optional exercise, you can explore completing your own Family Health Portrait using the My Family Health Portrait online tool. As you complete the printable form, you may find that you need answers you don’t have – giving insight into your own health, your family risk profile, and opportunities to improve the lives of those you care about.

NOTE: This is an optional learning exercise; you will not be required to submit any information related to your health portrait.
